Allocate approximately 1 to 1 1/2 hours for your appointment, including time with the therapist, and administrative tasks, such as completing paperwork, scheduling further appointments, and payment procedures.
As a new patient seeking physical therapy or a returning patient, you may have several questions regarding your first visit, what to expect during treatment, and how we operate. Below, you will find some frequently asked questions.
During your physical therapy evaluation, our therapists will make a movement analysis, assess your range of motion, strength, and joint mobility to understand your condition. Based on this evaluation, we will create a personalized treatment plan. If time allows, we may begin treatment that day, including teaching you a home exercise program and providing hands-on manual therapy if appropriate.
The initial appointment typically lasts around one and a half hours. This allows time for paperwork (unless completed in advance), evaluation, treatment, check-in, scheduling, and payment. Subsequent visits usually require 30-45 minutes.
Please bring your insurance card, a form of identification, and a prescription/referral from your doctor.
To enable us to assess your movement and access the site of your injury, wear comfortable clothing suitable for physical activity, such as gym attire or running clothes. Opt for loose clothing that can be easily moved aside to access the injured area. We recommend wearing comfortable, flat-soled shoes, preferably running or workout shoes.
We provide private treatment rooms for changing. Simply inform the front desk upon arrival, and they will guide you to a changing room.
Parking options include the garage with free validation, the area behind the building, and ample street parking. Please avoid parking in the museum lot. Additionally, there is a convenient loading zone right outside our door for drop-off and pick-up.
At Sports & Performance Physical Therapy, we are in-network with Medicare Part B and most major PPO insurance plans. Refer to our Insurance & Payment page for a complete list of accepted insurances.
For those paying for visits out of pocket, we offer competitive cash rates. Our pricing is as follows:
We can provide the necessary documentation, such as a super bill, for reimbursement from your insurance company. These rates do not reflect your insurance contractual bill rate.
Upon your arrival, check-in at the front desk and proceed with the following:
Check in with your name, arrival time, and the name of your physical therapist.
Make the required payment for co-pay, co-insurance, deductible, or visit fees.
Take a seat and wait for your physical therapist to escort you.
You will receive a personalized plan of care upon your physical therapist completing a comprehensive evaluation of your injury, surgery, or problem area. This may include a home exercise program, therapeutic exercises, manual physical therapy, or sports-specific activities. After your appointment, visit the front desk to:
Confirm and schedule any future follow-up visits based on your plan of care.

During follow-up visits, our clinicians will guide you through the next stage of your recovery, tailoring the treatment to help with your injury or condition. This may involve learning new exercises or receiving different manual therapies.
